You can purchase dental as a standalone product, or it can 
be added to a medical plan to boost an employee benefit package. 

The traditional plan design provides coverage for many in-network providers and offers a buy-up option to include coverage for out-of-network providers. 

Contributory and voluntary plans 

We offer both contributory and voluntary premium options. 

For contributory plans, the employer contribution must equal at least 50% of the single-coverage monthly premium. Employers with up to four eligible employees must have 100% participation after valid waivers. Employers with five or more eligible employees must have 75% participation after valid waivers. 

For voluntary plans, employers need at least 30% participation. There are also coverage waiting periods for certain services. 

Coverage waiting periods (voluntary plans) 

  • Preventive and Diagnostic: No waiting period 
  • Basic: 3 months 
  • Major: 12 months 
  • Missing Tooth: 36 months 

Tip: When dental plans are bundled with an existing medical plan, employers will be eligible for lower dental rates. 

* Dependents will lose dental coverage at the end of the month of their 26th birthday.

Delta Dental of Idaho Flex Plans

Delta Dental of Idaho offers employers flexibility in how they provide dental benefits. With the Flex Plan, the employee may choose whether to enroll in dental coverage and selects from one of four plan designs.

Groups must maintain a minimum of two enrolled employees.
More than 94% of Idaho dentists participate in the Delta Dental network, giving employees convenient access to the state’s largest network of providers.

Flex Plan Options

Employers may choose one of the following plans to offer:

  • PPO Preventive
    • No deductible
    • No waiting periods
    • Unlimited annual maximum
       
  • PPO $1,250
    • $50 individual / $150 family deductible (Basic and Major services only)
    • $1,250 annual maximum
       
  • PPO $1,500 with Child Orthodontia
    • $50 individual / $150 family deductible (Basic and Major services only)
    • $1,500 annual maximum
    • $1,500 orthodontia lifetime maximum (children only)
       
  • PPO $3,000 with Child & Adult Orthodontia
    • $50 individual / $150 family deductible (Basic 
and Major services only)
    • $3,000 annual maximum
    • $2,000 orthodontia lifetime maximum (children 
and adults)

Waiting Periods

  • No waiting period if enrolled during open enrollment
  • 12-month waiting period for late enrollees (applies to Major and Orthodontic services only)

Delta Dental


In Nevada, dental plans are offered through Delta Dental. 
This traditional plan design provides coverage for many 
in-network providers and offers employers the option to include coverage for out-of-network providers. 

Budget-friendly costs 
With a DeltaCare USA plan, employers will know copays 
and out-of-pocket costs before treatment begins. 

  • No deductible or maximums for covered services 
  • Pay only the copay (if any) at the time of treatment 

A partner in oral health 

The DeltaCare USA plan encourages regular dental care 
with an extensive list of covered services. 

  • Low or no copays for services like cleanings and exams 

Delta Dental: contributory only 

Employees waiving coverage will not be counted toward participation if they have other valid dental coverage. 

  • For employers with up to four eligible employees 
after valid waivers — 100% of eligible employees 
must participate. 
  • For employers with five or more eligible employees — 75% of eligible employees must participate.
