In addition to the routine eye exams covered by our medical plans, we offer coverage for vision hardware through EyeMed Vision Care. Benefits include contacts, frames, eyeglass lenses, and lens options.

Our voluntary and contributory plans offer flexible cost-sharing options designed for any budget.


Voluntary plans


Eligible employees can purchase coverage at 100% cost with no employer contribution.


Contributory plans


Costs are shared with employees. Contributions must be equal to at least 75% of the single coverage monthly premium.


Eligibility


Enrollment is optional at the employee-level with no required minimum enrollment percentage for participation. 

Eligible employees and their dependents may apply for coverage. Eligible dependents include:

  • The insured’s legal spouse
  • Children younger than age 26
  • Eligible disabled children older than age 26
  • Children under court-ordered legal guardianship until legal guardianship ends
