Select Health Value
Select Health Value is best for those members living in Box Elder, Cache, Davis, Iron, Morgan, Salt Lake, Summit, Tooele, Utah, Wasatch, Washington, and Weber counties. This network includes access to dozens of clinics and providers who are aligned with Intermountain Health, as well as Huntsman Cancer Institute for a cancer-related diagnosis.
Select Health Med
Covers the state of Utah with more hospitals and providers than the Select Health Value network. Includes Huntsman Cancer Institute for a cancer-related diagnosis and the Moran Eye Center. Benefits are available at out-of-network hospitals and providers for most services. Includes national access through UnitedHealthcare Options PPO network.

Select Health Preference gives you and your employees access to both Select Health Value and Select Health Med networks. Employees who reside outside of Utah, Idaho, Nevada, and Colorado can access the UnitedHealthcare Options PPO network.

Select Health Med is best for those living in Bannock, Bingham, Bonneville, Caribou, Cassia, Fremont, Jefferson, Madison, Minidoka, Power, and Teton counties, in addition to those looking for comprehensive coverage throughout Utah and access to some providers and facilities in Nevada.
BrightPath
The BrightPath network is best for those living in Eastern or Northern Idaho who would like access to BrightPath providers and facilities. Available to residents of Bear Lake, Bingham, Bonneville, Franklin, Fremont, Jefferson, Kootenai, Madison, Oneida, and Teton counties.
St. Luke’s Health Plans (SLHP)
The Select Health SLHP network is best for those living in Southwest Idaho who would like access to St. Luke’s providers and facilities. Available to residents of Ada, Adams, Blaine, Boise, Camas, Canyon, Cassia, Custer, Elmore, Gem, Gooding, Jerome, Lemhi, Lincoln, Minidoka, Owyhee, Payette, Twin Falls, Valley, and Washington counties.
Saint Alphonsus Health Alliance (SAHA)
The SAHA network is best for those looking for a network of SAHA providers and facilities who live in select counties in Southwest Idaho. Available for residents of Ada, Canyon, Boise, Gem, Payette, and Washington counties.

Select Health Med POS Select Health Med POS is available in Bannock, Bear Lake, Bingham, Bonneville, Cassia, Caribou, Franklin, Fremont, Jefferson, Madison, Minidoka, Oneida, Power, and Teton counties. With the addition of St. Alphonsus Health System, Select Health has expanded the coverage area to include Ada, Boise, Canyon, Gem, Owyhee, Payette, and Washington counties. Benefits are also available at out-of-network hospitals and providers for most services. This plan option also includes national access.

Select Health Value (HMO)
The Select Health Value network is highly integrated with Intermountain Health and provides access to providers and facilities throughout Clark and Nye counties. Primary Care Provider selection is required on this network, and referrals are required for specialty care.*

Select Health Med (POS)
The Select Health Med network provides access to providers and facilities throughout Clark and Nye counties in Nevada and all of Utah and Idaho. This plan option also includes national access. Select Health does not require referrals on this plan.
Tip: Using in-network providers and facilities where possible maximizes plan benefits and saves employees money.
NOTE: Dependents living outside Utah, Idaho, Nevada, and Colorado can access in-network benefits through the UnitedHealthcare Options PPO network.

Select Health Value
Select Health Value is a great option for members living in Adams, Arapahoe, Boulder, Broomfield, Clear Creek, Delta, Denver, Douglas, Elbert, El Paso, Gilpin, Jefferson, Larimer, Mesa, Park, Pueblo, Routt, Teller, and Weld counties. This network includes access to AdventHealth, Boulder Community Health, Children’s Hospital Colorado, Intermountain Health, National Jewish Health, and UCHealth. This network does not include Out-of-Network (OON) coverage.
Select Health Value POS
Select Health Value POS encompasses the 19 counties included on the Value network with access to MultiPlan outside of the Value footprint. Benefits are available at out-of-network hospitals and providers for most services. This plan option also includes national access.
