Demographic changes

There are three ways to make demographic changes:

  • Make the appropriate change in Link
  • Submit a Change Form directly to Select Health
  • Have the member log in and make changes in their Select Health account

Visit selecthealth.org/forms.

Discontinuing coverage

Employees may be removed from the group plan due to:

  • Termination of employment
  • Retirement
  • Death
  • Leaving for active military service
  • Loss of eligibility (full-time to part-time, but still employed)
  • Termination of continuation coverage (such as COBRA)
  • Waiving coverage (e.g., due to coverage through a spouse or parent plan)

If employees and/or dependent(s) lose eligibility, in most cases coverage will terminate at the end of the month in which the loss of eligibility occurred.
For faster service, please submit changes and terminations through Link. If you have any questions, please contact our Sales team at 844-442-6294.
