Demographic changes
There are three ways to make demographic changes:
- Make the appropriate change in Link
- Submit a Change Form directly to Select Health
- Have the member log in and make changes in their Select Health account

Discontinuing coverage
Employees may be removed from the group plan due to:
- Termination of employment
- Retirement
- Death
- Leaving for active military service
- Loss of eligibility (full-time to part-time, but still employed)
- Termination of continuation coverage (such as COBRA)
- Waiving coverage (e.g., due to coverage through a spouse or parent plan)
If employees and/or dependent(s) lose eligibility, in most cases coverage will terminate at the end of the month in which the loss of eligibility occurred.
